Objective: To assess parental awareness, knowledge, attitudes and practices regarding pit and fissure sealants, and to identify the factors associated with sealant uptake in their children.

Methods: Cross-sectional survey of 174 parents using an 18 questionnaire. Six items were scored as a knowledge scale (0–6) and three as an attitude scale (0–6). Associations were tested with Pearson's chi-square, Fisher's exact test, Mann–Whitney U and Kruskal–Wallis H, with Benjamini–Hochberg correction across the primary family of tests. Independent predictors of sealant uptake and of willingness were identified by multivariable binary logistic regression. Two-sided α = 0.05.

Results: Only 52.9% (95% CI 45.5–60.1) had ever heard of sealants and just 29.9% identified caries prevention as their purpose. Mean knowledge score was 2.23 ± 1.79 of 6, 60.3% scored in the poor range. Sealants had been received by 37.4% of children. Uptake was strongly associated with dentist recommendation (65.6% vs 22.1%, OR 6.70, 95% CI 3.36–13.37, p < 0.001) and with regular dental check-ups (61.8% vs 21.8%, OR 5.78, 95% CI 2.85–11.71, p < 0.001). In the adjusted model, dentist recommendation (aOR 4.43, 1.93–10.18) and regular check-ups (aOR 3.11, 1.39–6.95) remained independent predictors, while knowledge and attitude scores did not (model χ²(5) = 53.46, p < 0.001, Nagelkerke R² = 0.395, AUC = 0.82).

Conclusion: Parental knowledge of pit and fissure sealants is poor, and uptake is driven far more by contact with and advice from a dentist than by what parents know or believe. Professional recommendation during routine visits is the most actionable lever for increasing sealant use.
